    Common Dosage Forms:

    Capsules: 150 mg, 300 mg, 600 mg

    Tablets: 300 mg

    Tablets, extended-release: 300 mg, 450 mg, 600 mg

    Oral Solution: 8 mEq per 5 mL equivalent to 300 mg

    FDA Indications/Dosage:

    For the treatment of manic episodes of manic-depressive illness: Initial dosage for acute mania is 600 mg given three times a day. The usual maintenance dosage is 300 mg given 3-4 times a day. Desirable maintenance serum levels of lithium are 0.6-1.2 mEq/L. Sustained-release tablets are usually given twice a day.

    Monitor: Serum lithium

    Pharmacology/Pharmacokinetics: The exact mechanism of action of lithium in the treatment of mania is unknown. Its mood-stabilizing may be due to an antagonism of the release of norepinephrine and dopamine from nerve terminals or from increasing the reuptake and inactivation of catecholamines. Lithium does not inhibit the release of serotonin. Another potential mechanism of action is through depletion of phosphatidylinositol in selected cells in the CNS. They may increase the responsiveness of neurons to various neurotransmitters. Peak plasma levels are reached in 2-4 hours with a half-life of approximately 24 hours. Excretion occurs in the urine.

    Drug Interactions: Use with carbamazepine may produce neurotoxicity. May cause hypothyroidism when used with iodides. NSAIDs, ACE inhibitors, and thiazides may increase serum levels. Urinary alkalinizes may decrease serum levels. Variable toxic effects may be seen when used with HALOPERIDOL.

    Contraindications/Precautions: Contraindicated in patients with significant renal or cardiovascular disease, severe debilitation, dehydration, or sodium depletion. Use with caution in patients taking diuretics or ACE inhibitors and during pregnancy or lactation. Lithium decreases sodium reabsorption which may lead to sodium depletion and lithium toxicity. Adequate sodium and fluid (2500-3500 mL) intake should be maintained. Lithium has a narrow therapeutic index. LITHIUM TOXICITY IS CLOSELY RELATED TO SERUM LITHIUM LEVELS. Pregnancy Category D.

    Adverse Effects: Han tremor, nausea, and mild thirst may occur during initial therapy. Toxicity is manifested by diarrhea, vomiting, drowsiness, muscular weakness and lack of coordination followed by blurred vision, tinnitus, and extreme polyuria.

    Patient Consultation:

    Closely follow recommended dosage.

    If a dose is missed, take it as soon as possible. If it is closer to the time of your next dose than the dose you missed, skip the missed dose and return to your dosing schedule. Do not double doses.

    Effects may not be seen at once—may take 1-2 weeks before optimum effect is seen.

    Maintain your normal diet and have a fluid intake of 2-3 quarts a day during therapy.

    Avoid overexposure to heat and sweating during therapy.

    Store in a cool, dry place away from sunlight and children.

    Contact a physician if the above side effects are severe or persistent.
